Reimbursements for Travel
For all applicants/non-UW people requesting travel reimbursements, attach a copy of your receipts and email this information to Su Tipple (tipple@uw.edu) and Michelle Townsend (mtown@uw.edu):
1. Name (First and Last)
2. mailing address for your reimbursement check (if you bank outside the US, let Su know so she can send you a wire form to fill out instead)
If you are Canadian, we need 1 and 2 plus
3. a scan of your passport
If you are non-US but have a US green card, we need 1 and 2 plus
4. a scan of your green card
If you are any other citizenship than US or Canadian and do not have a US green card we need 1, 2 , and 3 plus
5. a scan of your visa
6. a copy of your most recent I-94 from entering the US, which you can get at https://i94.cbp.dhs.gov/I94/#/recent-search
